Bone Cancer

Bone cancer develops in bones like spine, the pelvic bones, legs, or arms (extremities). Bone cancer can be divided into two types:

 

1.Primary bone cancer, this type of cancer is caused by the body cells that grow abnormally and multiply until forming a cancerous tumor. Primary bone cancer can be found in both child and adult. The patient frequently visit the doctor by feeling the lump or mass on the body, much pain from broken bone or incidentally detection of the disease such as osteosarcoma
 
2.Secondary Bone Cancer/Bone metastasis
This cancer type is often found in patient with existing cancer such as breast cancer, lung cancer, prostate cancer, kidney cancer or multiple myeoloma and these cancers spread to the bone. The symptom does not improve from initial treatment, hence, a visit with a bone cancer specialist is recommended for further evaluation.
 
Symptoms of Bone Cancer
Bone cancer can begin in any bone in the body, but it most commonly affects the spine which can result in broken spine. The bone tumor also compresses the spinal cord or nerve roots. The patient may feel the lump and painful on the back that radiates down the arm and the leg. In addition, numbness and weakness of the arm and the leg can also be found. In case of delay treatment, it can lead to bladder or bowel incontinence which the patient may have difficulty in controlling the passage of urine or stool.
 
Another part of the bone that cancer often develop is pelvic and extremities. The patient normally visits the doctor due to painful symptoms which occur at night or the pain from movement. The patient may sometimes feel the lump. If left untreated, the bone can be broken and when the bone is already broken, the treatment outcome will not be effective particularly when the patient has primary bone cancer.
 
1. Feeling the lump under the skin
2. Bone pain from having cancer such as breast cancer, lung cancer. The bone pain does not improve, it is a chronic or persistent ache.
3. Considerable pain caused by bone, weakness of legs and arms
4. Unexplained weight loss and fatigue
5. Incidental discovery of the bone cancer
 
How to diagnose Bone Cancer
Initially, the physician will do history taking, physical examination and investigations such as the following:
 
        -    Plain X-rays
        -    MRI Scan 
        -    CT Scan 
        -    Bone scan or PET/CT scan
        -    Biopsy
 
Besides bone cancer, the diagnosis can be as following:
 
        -    TB / Bacterial Spondylitis or Osteomyelitis
        -    Spinal cord tumor
        -    Benign bone tumor / Tumor like masses)
        -    Cervical Spondylosis or Lumbar Spondylosis
        -    Osteoporotic Vertebral Fracture
        -    Soft tissue tumor
        -    Others such as Paraspinal Cyst / Bursitis), Paraspinal nerve sheath tumor), Vascular tumor
 
Treatment of bone cancer
The treatment of bone cancer depends on the type and stage of the disease
 
1.In case of primary malignant bone tumor, if it is detected early which means bone cancer still does not spread to other parts of the body; the surgical treatment could be successful performed. Chemotherapy and radiation therapy will be given based on each individual’s condition together with the removal of cancerous tumor or growth from the body.
 
2.In case of bone metastasis, the goal of treatment is increasing quality of life, reduce bone pain and reduce complications allowing self-care as much as possible.

The first bone surgery is the most important
Concerning primary bone cancer, the appropriate surgical technique can successfully treat the cancer. However, if the surgery is wrongly performed, the risk that cancer will recur is high. Furthermore, there is almost no chance of revision surgery owing to contamination of surgical area.
