Phyathai Co-Morbid Index

To ensure the utmost safety for all our surgical patients, we have developed the 'Phyathai Co-Morbid Index,' a comprehensive pre-operative risk assessment tool. Orthopedic surgery is a delicate procedure with profound implications for the body, and at the Phyathai 2 Hospital Orthopedic Institute, we understand the importance of precision. Our Phyathai Co-Morbid Index helps us identify potential risks and optimize surgical outcomes. This is just one example of our commitment to providing world-class care.

To minimize risks in high-risk patients and ensure the best treatment outcomes, Professor Dr. Thanainit Chotanaphuti, Head of the Orthopedic Institute and an orthopedic surgeon with over 20 years of experience, collaborated with a team of doctors and nurses to develop a quality control program called the Phyathai Co-Morbid Index. This is a Quality Service initiative applied to all patients undergoing orthopedic surgery.

'The principle involves evaluating patients based on a three-part assessment, conducted jointly by the medical and nursing teams. If the total score is between 6 and 7, the surgery is considered safe and can proceed immediately. However, if the score is 8 or higher, the patient is deemed at risk, and the case will proceed to an MDT Conference (Multi-Disciplinary Conference). In this process, a team of specialists, nurses, and relevant staff will meet to evaluate the options and potential impacts of each choice. This allows the patient and their family to fully understand and anticipate the outcomes before making a decision. This index is exclusively available at Phyathai 2 Hospital,' explained Rungthip Namprom, a professional nurse and coordinator."

The development of the Co-Morbid Index was driven by Phyathai 2 Hospital's desire to achieve the most optimal treatment outcomes for every patient. Therefore, if a patient is found to be at risk, an MDT Conference (Multi-Disciplinary Team) will be held. This conference involves the attending physician, internal medicine specialists, nurses, the OPD team, the ward team, and the nurse case manager. Together, they discuss the risks and success rates based on JCI (Joint Commission International) standards. After evaluating the situation, the team will summarize the findings and inform the patient, their relatives, and family about the level of risk, the available options, and what might happen if surgery is not chosen.

If a physician deems the patient at risk but has not discussed the case with the family or held a team meeting, Professor Dr. Thanainit will strictly prohibit the scheduling of surgery. This process, driven by the doctor, is intended to ensure that every team member prioritizes patient safety above all else.
