Gynecologic Laparoscopic Surgery

The diseases that most women are concerned are diseases associated with menstruation, reproductive system and vagina. To treat these diseases, if it is not severe, you may only take medications as prescribed by physician. However, many cases require surgical treatment which the physician mostly chooses the open surgery considered as traditional approach. For traditional surgical technique, a single large incision is made on the abdomen depending on the type of surgery and this makes patients suffer from wound. The surgery recovery is also slow and there is a high chance that the wound become infected and the wound may split. In addition, open surgery results in significant scar tissue forming around the incision area (abdomen) which make many women scared of gynecologic surgery.

 
Today, laparoscopic surgery is another treatment option of women who must undergo the surgery since the technology of laparoscopic surgery provides decreased complications, less pain due to small incisions and shorter recovery time. Therefore, laparoscopic surgery nowadays receives a considerable attention.
 
What is gynecologic laparoscopic surgery?
Laparoscopic surgery of women or gynecologic laparoscopic surgery can be divided into two types
 
1. Laparoscopy which is used to examine the organs in the abdomen especially in the pelvic floor, using laparoscopy to examine a woman's pelvic organs for diagnosis and for surgery.
2. Hysteroscopy is using a hysteroscope, a thin, lighted tube that is inserted into vagina to examine any abnormalities in the uterus and the surgery can be performed to remove submucosal fibroid without making a large incision on the abdomen and uterine wall.

   -    Leiomyoma
   -    Cervical cancer (initial state)
   -    Endometrial cancer (initial state)
   -    Ovarian cyst
   -    Ectopic Pregnancy

The diseases that cause chronic abdominal pain
   -    Endometriosis
   -    Ovarian tumor and uterine fibroids
   -    Ectopic pregnancy
   -    Interval Tubal Sterilization
   -    Treatment of infertility diseases such as removing blocked fallopian tubes and reconnecting them
   -    Treatment of diseases of the uterine cavity
 
How to perform gynecologic laparoscopic surgery
The team of gynecologist will make small incisions with size approximately 0.5-1 centimeter on the abdomen or near the organs that require surgery. One to four incisions are made depending on the type of diseases to allow the insertion of a thin, lighted tube attached with camera which will record the internal organs of patients clearly. The camera transmits an image of the internal organs onto a television monitor which gives the surgeon a clear view of a surgical area. Consequently, the surgical outcome is more accurate and precise.
 
Instruction for patients after undergoing gynecologic laparoscopic surgery
1. Follow the physician’s advice strictly
2. When the doctor allows you to eat, drink lots of water, and do not hold urine
3. Get up and walk often to prevent adhesions
4. When a problem or abnormality occurs, consult a doctor or nurse
5. Visit the doctor for a physical examination and pelvic examination after surgery on the day and time appointed by the doctor.

How is gynecologic laparoscopic surgery better than traditional surgery?
1. The patient has small wound compared with traditional surgery. There are 3-4 wounds with size 0.5-1 centimeters on the abdomen
2. Due to small wound, pain after surgery is less than traditional surgery; and there is no need to use strong painkillers.
3. The patient can walk within 1 day after surgery, and able to carry out personal routines. They are more comfortable and can leave the hospital to recover at home faster than before. Most patients stay in the hospital 1-3 days after surgery.
4. Work absence is not long like the traditional approach. Patients recover at home for only 1 - 2 weeks and can return to work as usual.
5. Postoperative adhesions and complications are less common than with traditional techniques.
 
Limitation of gynecologic laparoscopic surgery
1. Gynecologic laparoscopic surgery cannot be performed with every patients such as patients with large tumor over 15 centimeters 
2. Patients with underlying disease such as leakage in the diaphragm, lung disease or heart disease. Gynecologic laparoscopic surgery cannot be performed on these mentioned patients.
3. Patients with excessive abdominal or pelvic adhesions. For example, patients who have had abdominal or pelvic infections before or patients who have had abdominal surgery many times.
4. Doctors who perform laparoscopic surgery must specialize in laparoscopic surgery and well trained to perform laparoscopic surgery.
